Insurance Information
We have expertise dealing with many different insurance companies. Some companies offer many different dental and medical plans. Some companies change benefits, co-pays and deductibles faster than we (or you) can keep up with. We do our best to provide you with accurate coverage estimates based on information available to us. At times, it is almost impossible to accurately estimate our patients' insurance co-payment. Dealing with some insurance companies can be difficult and time consuming. We will do our very best to stay informed and current with your coverage, and we ask that you notifty us immediately of any changes to your insurance.
After the initial examination, we will file the insurance claims for you, if you wish. Many times we can utilize electronic, e mail and fax submission to greatly reduce the waiting time for both pre-determination and payment.
Co-Payment
Most insurance policies cover between 20-80% of periodontal treatment, and usually are limited by an annual maximum. After receiving the predetermination we will inform you of your co-payment amount. At that time a payment schedule can be arranged, but all co-payment is due by the final treatment visit.
Private & Group Insurance
As a courtesy to our patients with medical and/or dental benefit plans, we will submit necessary claim forms, receipts, and other information to your insurance company.
Upon receipt of an insurance payment, any balance due will be billed to you. If you have deposited an excessive co-payment, it will be refunded to you.
